Liability Waiver
By registering with Sequences Wellness Centre, you are in agreement:
* That you are participating voluntarily in a treatment, therapy session, online or offline class, course, workshop offered by Sequences Wellness.
* You recognize that Physiotherapy sessions requires The Patient to expressly states hereby that he/she will be voluntarily receiving the physiotherapy treatment referred to in the Patient’s Intake form and the Informed Consent to which this waiver and release is attached and the Patient hereby assumes all risks of injury or every nature whatsoever with might result from the receipt of this physiotherapy treatment at Sequences Wellness Centre. The Patient hereby waives and releases any and all claims that he/she has or may have against the Sequences Wellness Centre, its employees or agents for injury sustained by the Patient as a result of the receipt of the physiotherapist assessment and treatment to be performed by the Registered Physiotherapist. The Patient hereby acknowledges that he/she has carefully read this waiver and release and fully
understands that it is a waiver and release of liability of the Sequences Wellness Centre and agrees that such a waiver and release is reasonable and proper based on the nature of Sequences Wellness Centre’s business.
The Patient hereby waives and releases Sequences Wellness Centre from any claims of every nature or kind whatsoever that he/she may have against Sequences Wellness Centre with respect to any loss or theft of personal property in respect of the facilities operated by Sequences Wellness Centre.
* You recognize that Massage Therapy sessions requires that if you experience any pain or discomfort during the session, you will immediately inform the therapist so that the pressure and/or strokes may be adjusted to your level of comfort. You further understand that Massage Therapy should not be construed as a substitute for medical examination, diagnosis, or treatment and that You should see a physician, chiropractor, or other qualified medical specialist for any mental or physical ailment of which you are aware. Because Massage should not be performed under certain medical conditions, You affirm that You have stated all your known medical conditions, and answered all questions honestly. You agree to keep the therapist updated as to any changes in your medical profile and understand that there shall be no liability on the therapist’s part should You fail to do so. This is a Therapeutic Massage session and any sexual remarks or advances will terminate the session and You will be liable for payment of the scheduled treatment. You understand the Massage Therapist practitioner reserves the right to refuse services to you for any reason that she deems necessary.
* You recognize that Movement Therapy sessions requires physical exertion that may be strenuous and may cause physical injury, and you are fully aware of the risks and hazards involved. You represent and warrant that you are physically fit and you have no medical condition that would prevent your full participation and you agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which you might incur as a result of participating in the program.
* You understand that you may receive physical or verbal assists or adjustments to enhance or correct your body posture during session by either the teacher or an assistant in class. You will take accountability for alerting the teacher and assistant of any injury or impairment in advance before session begins or if you do not want to receive any assists. You can request for an assist coaster at the reception.
* You understand that Sequences Wellness reserves the right to make alterations to the timetable, instructors and/or equipment, terms and conditions as well as to those ancillary facilities (e.g. showers), provided to members and on occasion with minimal notice.
* You understand that photos and videos may be taken of the class and used by Sequences Wellness.
* You knowingly, voluntarily, and expressly waive any claim you may have against Sequences Wellness for injury or damages that you may sustain as a result of participating in the program.
* You, your heirs, or legal representatives forever release waive, discharge, and covenant not to sue for any injury or accident caused by their negligence or other acts.
* You understand that an area for personal belongings is available during class, however, you agree that Sequences Wellness is in no way responsible for the loss or damage of any of your belongings while you attend class.
Cancellation Policy
Physiotherapy Cancellation Policy:
Please allow 6 hours notice for cancellations or reschedules.
Cancellations within 6 hours will incur 100% charge.
Massage Therapy Cancellation Policy:
Please allow 6 hours notice for cancellations or reschedules.
Cancellations within 6 hours will incur 100% charge.
Movement Therapy Cancellation Policy:
Please allow 3 hours notice for cancellations or reschedules.
Cancellations within 3 hours will incur 100% charge.
Purchase Policy
* All Purchases will have a defined limitation for the expiration of credits associated with them, normally is 2-3 months but some options may vary.
* Please not that all Treatments, Session, Packages, Workshops, Trainings and Products purchases are final, non-refundable and cannot be extended.
* In any case of an proven emergency or delay on our end, purchases can be discussed with management for extension.
Terms & Conditions
Bookings:
We recommend booking your treatment or session in advance to ensure you have a spot in class. Appointment is confirmed only after completing payment.
Health Concerns:
Please inform us in case of any health concerns, or injuries which could impact your service when booking a class.
Movement Therapy Sessions Late Arrivals:
If you arrive 15minutes late, you will not be permitted to attend session, in order to not disturb the flow of the session for teacher and clients.
Physiotherapy & Massage Treatments Late Arrivals:
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for “full cost” of the cost of session. Out of respect and consideration for your therapist and other customers, please plan accordingly and be on time.
